Telemedicine in Patients with Type 2 Diabetes

Introduction: Type 2 diabetes is a chronic disease requiring continuous monitoring, education, and behavioral support. Telemedicine has expanded as a means of delivering remote care to these patients, particularly after the COVID-19 pandemic, yet evidence on its effectiveness is inconsistent. Methods: This narrative review was based on an examination of 12 studies published between 2023 and 2026. Sources included systematic reviews, meta-analyses, narrative and scoping reviews, and two clinical trials, mainly retrieved from PubMed and thematically categorized. Findings: Most meta-analyses showed that telemedicine reduces HbA1c compared with usual care; for example, mean reductions of 0.38 units across 58 trials and 0.59 units across 12 pandemic-era trials were reported. These interventions improved self-management and medication and dietary adherence, and some studies also reported better fasting glucose, blood pressure, and insulin resistance, whereas effects on weight and lipids were inconsistent. In contrast, one primary-care meta-analysis and one Indian meta-analysis found no significant difference from usual care or in-person visits. Nurse telephone follow-up also improved self-care and HbA1c. Clinical, individual, organizational, and technical challenges, including low digital literacy and limited reimbursement, were major barriers. Conclusion: Telemedicine can improve glycemic control and self-management in type 2 diabetes and is at least comparable to usual care, although heterogeneity across studies is high. Addressing technical and educational barriers and conducting long-term studies are essential.
