The Role of the Internet of Medical Things (IoMT) in the Development of mHealth and Telemedicine Applications

چکیده
Abstract: Background: The Internet of Medical Things (IoMT) fundamentally transforms mHealth and telemedicine, shifting healthcare toward proactive “hospital-at-home” models. This study evaluates IoMT’s impact, focusing on technologies like 5G and Edge/Fog AI, while addressing privacy, security, and interoperability challenges. Method: A PRISMA-compliant hybrid review searched five major databases (PubMed, Scopus, Web of Science, IEEE Xplore, Google Scholar) from 2015 to early 2025. Out of 412 initial records, 21 high-quality studies met rigorous criteria to analyze technological infrastructure, clinical efficacy, security/privacy, and integration challenges. Findings: IoMT significantly enhances clinical efficacy in remote patient monitoring and chronic disease management by enabling early deterioration detection. Technologically, a critical shift from centralized clouds to Edge/Fog computing and 5G networks minimizes latency for real-time processing. However, challenges persist, including interoperability barriers, sensor power limitations, and the complex trade-off between robust security (Blockchain, AES) and device performance. Conclusion: IoMT-integrated telemedicine provides a highly effective, patient-centric framework. To overcome current barriers, this study proposes a multi-tiered architecture (perception, edge/fog, network, cloud). Future progress requires standardized protocols, ethical AI, Lightweight Cryptography (LWC), and Federated Learning to balance security with real-time performance.
