چکیده
Background: The decline in fertility and the recent pronatalist turn in population policies in Iran have highlighted the role of health-care providers, especially midwives, in fertility counselling and shaping couples’ attitudes toward childbearing. Aim: This study aimed to assess the attitudes toward childbearing and related factors among midwives working in comprehensive health centers in Isfahan, Iran. Methods: This descriptive-analytic cross-sectional study was conducted among all midwives working in comprehensive health centers in Isfahan city. A census sampling approach was used, and finally 152 midwives who met the inclusion criteria were included in the analysis. Data were collected using a two-part questionnaire comprising sociodemographic and occupational characteristics and a researcher-developed Attitude toward Childbearing Questionnaire (21 items on a five-point Likert scale). Content validity was confirmed using the Content Validity Ratio (CVR) and Content Validity Index (CVI). Internal consistency was acceptable with a Cronbach’s alpha of 0.78, and temporal stability was confirmed by a test-retest correlation above 0.8. Data were entered into SPSS and analyzed using descriptive statistics, one-way analysis of variance, and Tukey’s post-hoc test at a significance level of 0.05. Results: The mean total attitude score toward childbearing was 79.86 ± 5.96, and the mean item score on the 1-5 scale was 3.8 ± 0.28, indicating a relatively positive attitude toward childbearing among the midwives. Mean attitude scores differed significantly by economic status, years of work experience, and employment status (p<0.05). The highest mean scores were observed among midwives with poor economic status (approximately 4.10) and those with 15-20 years of work experience (about 4.19). Midwives with non-permanent employment had higher mean attitude scores than those with permanent contracts (about 3.91 vs. 3.76). In contrast, no significant associations were found between attitude scores and age, marital status, number of children, number of siblings, or educational level (p>0.05). Conclusion: Overall, midwives working in comprehensive health centers in Isfahan had relatively positive attitudes toward childbearing. However, the strength of these attitudes varied more by occupational characteristics and perceived economic status than by classical individual factors such as age and education. When designing educational and counselling interventions on childbearing, it is recommended to consider midwives’ lived experiences and working conditions and to strengthen their role as knowledgeable counsellors and advocates for reproductive rights.
