چکیده
Introduction: Human papillomavirus (HPV) infection is highly prevalent during the reproductive years and may alter assisted reproductive technology outcomes through effects on sperm quality, fertilization, embryo development, and implantation. This study reviewed the impact of HPV infection on reproductive outcomes among infertile couples undergoing assisted reproductive treatment. Information & Methods: In this narrative review, PubMed, Scopus, Web of Science, Embase, the Cochrane Library, PsycINFO, SID, Magiran, IranMedex, and Google Scholar were searched through July 2026. Observational studies evaluating HPV infection in the woman, the man, or both partners in relation to intrauterine insemination, in vitro fertilization, or intracytoplasmic sperm injection outcomes were included. Fourteen studies were analyzed. Findings: Four of the 14 studies reported significantly lower pregnancy or live-birth rates in HPV-positive participants, whereas 10 found no significant difference. Of 12 studies evaluating miscarriage, three reported a significant increase and nine found no significant association. The only study addressing preterm birth reported a higher rate among HPV-positive women; however, no independent association remained after adjustment for confounders. Differences in sampling site, genotype, diagnostic method, treatment type, and sample size may explain the heterogeneous findings. Conclusion: Current clinical evidence is insufficient to establish an independent and definitive adverse effect of HPV infection on assisted reproductive technology success. Well-designed prospective studies with adequate sample sizes, genotype and viral-load assessment, and rigorous control of confounding factors are required.
