Nurse-Led Transitional Care Interventions and Outcomes in Older Adults After Hospital Discharge: A Scoping Review on Readmission and Quality of Life

چکیده
Aims: Hospital discharge is a critical transition for older adults and may increase the risk of fragmented care, medication-related problems, and difficulties in self-management. This scoping review aimed to map evidence on nurse-led transitional care interventions and their effects on hospital readmission and quality of life among older adults after discharge. Method: This scoping review followed the Arksey and O’Malley framework, Joanna Briggs Institute guidance, and PRISMA-ScR reporting principles. Eligible studies included older adults discharged to home or community settings who received transitional care interventions led, coordinated, or substantially delivered by nurses and reported readmission, quality of life, or other post-discharge outcomes. Studies focusing on inpatient-only care, non-nursing interventions, younger populations, or unrelated outcomes were excluded. Evidence included randomized controlled trials of nurse-coordinated transitional care for frail older adults. A systematic review across MEDLINE, CINAHL, PubMed, Scopus, Embase, and Cochrane Library identified 7,945 abstracts and 16 eligible randomized controlled trials. Data were charted according to population, intervention characteristics, delivery methods, and outcomes. Findings: Nurse-led transitional care commonly involves discharge preparation, education, care coordination, follow-up, and self-management support. Evidence suggests potential reductions in hospital readmissions and improvements in continuity of care and quality of life, although findings vary according to intervention characteristics and outcome measures. Conclusion: Nurse-led transitional care may improve post-discharge outcomes in older adults, particularly by supporting continuity of care and reducing avoidable readmissions. Further research is needed to identify the most effective intervention components and implementation strategies.
