چکیده
Background: Retention of women living with HIV in prevention of mother-to-child transmission (PMTCT) programs and timely infant HIV testing remain major challenges in high-burden settings. Remote communication technologies, including SMS and phone calls, have been proposed to improve retention. This study aimed to systematically review randomized controlled trials. Methods: A systematic search was conducted in PubMed, Scopus, and Web of Science using terms related to HIV/PMTCT, remote communication technologies, and retention/adherence. Eligible studies were randomized controlled trials evaluating remote communication interventions among pregnant or postpartum women living with HIV compared with usual care; secondary reviews, qualitative studies, non-randomized designs, and unrelated populations were excluded. Study quality was assessed using the Cochrane Risk of Bias 2 (RoB 2) tool. Finding: A total of 510 records were identified (Scopus=190, PubMed=118, Web of Science=202). After removing 185 duplicates, 325 records were screened. Sixty-two articles underwent full-text assessment, and 8 randomized controlled trials from 3 countries; Kenya (6), Uganda (1), and South Africa (1) involving more than 5,400 women were included. No trial had an overall low risk of bias; 6 (75%) had some concerns and 2 (25%) had a high risk of bias. Only 2 trials (25%) demonstrated significant improvement in retention/infant HIV testing, whereas 4 larger trials (50%) found no significant difference, and 2 small pilot trials (25%) were underpowered. Conclusion: Available evidence does not demonstrate consistent effectiveness of remote communication technologies for improving retention in PMTCT care. Caution is warranted when generalizing findings from early trials, and targeted interventions for high-risk subgroups should be further investigated.
