Digital Health Interventions and Telemonitoring for Self-Care and Hospital Readmission in Patients With Heart Failure: A Scoping Review

چکیده
Background: Heart failure is associated with a high symptom burden, impaired quality of life, frequent hospitalizations, and substantial healthcare use. Digital health interventions and telemonitoring may support self-care, facilitate early recognition of clinical deterioration, and prevent avoidable hospital readmissions. This scoping review mapped the available evidence on digital health interventions and telemonitoring for self-care and hospital readmission among patients with heart failure. Method: This scoping review followed the Arksey and O’Malley framework and was reported according to PRISMA-ScR guidelines. A systematic search was conducted in PubMed, Scopus, Web of Science, and CINAHL for English-language studies published from 2014 to 2026. The search combined controlled vocabulary and keywords related to heart failure, digital health, telemedicine, telemonitoring, mobile health, self-care, and hospital readmission. Eligible quantitative, qualitative, and interventional studies examined digital health or telemonitoring interventions in adults with heart failure and reported self-care, clinical-management, or readmission outcomes. Data were extracted and synthesized descriptively and thematically. Finding: Interventions included remote physiological monitoring, telephone- or video-based follow-up, mobile applications, web-based platforms, and technology-supported education. These interventions were commonly associated with improved symptom monitoring, self-care behaviours, medication adherence, and patient engagement. Multicomponent programmes integrating remote monitoring with clinical feedback, self-management education, and timely care-team responses showed more consistent potential for reducing hospital readmissions than stand-alone digital education. However, findings varied by intervention intensity, patient engagement, clinical integration, and outcome measurement. Conclusion: Digital health and telemonitoring may strengthen self-care and reduce potentially avoidable readmissions in heart failure. Further high-quality, long-term studies should identify sustainable and effective intervention components. Keywords: Heart Failure, Digital Health, Telemedicine, Self Care, Patient Readmission.
