چکیده
Background: Children and adolescents with cancer face significant psychological distress, necessitating the integration of developmentally appropriate, non-pharmacological interventions into routine clinical care. To evaluate the comparative effectiveness of mandala art therapy versus standard psychosocial interventions in reducing anxiety among pediatric cancer patients. Methods: A systematic search was conducted in international databases including PubMed, Scopus, Web of Science, Embase, CINAHL, and ProQuest from inception. Search terms were a combination of the following, using Boolean operators (AND/OR): "Mandala" OR "Mandala Art Therapy" OR "Mandala Coloring" AND "Cancer" OR "Neoplasms" OR "Oncology" AND "Child*" OR "Adolescen*" OR "Pediatric*" AND "Anxiety" OR "Psychological Symptoms" OR "Depression" OR "Distress". This review synthesized evidence from randomized controlled trials and quasi-experimental studies using a comparative framework to assess therapeutic efficacy, methodological variations, and age-stratified outcomes across international clinical settings. Results: Mandala-based and expressive art interventions significantly reduced anxiety symptoms in pediatric populations, with ten studies reporting statistically significant improvements. Mandala painting specifically outperformed alternative activities such as Sudoku in anxiety reduction, while procedural distress was notably alleviated through creative arts. Methodological heterogeneity was prevalent, with studies varying widely in session frequency, duration, and use of standardized control groups. Conclusion: While mandala art therapy proved to be a feasible and effective tool for anxiety management, the lack of standardized protocols limited the strength of clinical conclusions. Rigorous, large-scale trials are needed to establish evidence-based guidelines for integrating creative arts into pediatric oncology practice.
