چکیده
Background: Earthquakes can pose multiple challenges to the disaster management and delivery of health services. This study aimed to identify the challenges, requirements, and strategies associated with health system management and response to the Kermanshah earthquake. Method: A scoping review search was conducted in Scopus, PubMed, and Web of Science using keywords related to the Kermanshah earthquake, challenges, strategy, disaster management, and health system response for the period 2017-2026. Studies published in Persian or English that addressed the health or management dimensions of the Kermanshah earthquake, were considered eligible. Studies focusing on other disasters, purely geological or engineering aspects, reports lacking a clear methodology, and duplicate publications were excluded. Of 1,045 records initially identified, 56 relevant studies were selected for further assessment. Following independent screening by two researchers and resolution of disagreements by a third researcher, 30 studies were included in the final analysis. Data were analyzed using thematic analysis. Finding: The findings from the studies can be grouped into three categories: challenges related to infrastructure and physical facilities, including extensive damage to healthcare centers and water networks, issues with waste and debris management, and the need for field hospitals and structural reinforcement; challenges concerning human resources and capacity building, encompassing a lack of specialized training for nurses and staff, volunteer management issues, inadequate clinical preparedness, and ethical concerns; and challenges regarding coordination and resource allocation, covering a lack of inter-organizational integration, inequitable aid distribution, weaknesses in the logistics of medication and blood supplies, and the absence of unified response structures. Conclusion: Strengthening disaster preparedness, enhancing inter-organizational coordination, empowering human resources, and implementing integrated resource management strategies may contribute to improving health system response to disasters and enhancing the efficiency of resource utilization
