چکیده :
Background: Disasters increasingly disrupt women’s reproductive health through direct damage to health systems and indirect effects on fertility, pregnancy, and access to essential services. However, evidence remains fragmented across disciplines and outcomes. Objective: To synthesize the literature on women’s reproductive vulnerability in disasters, with emphasis on fertility, pregnancy outcomes, and access to reproductive health services. Methods: A narrative review was conducted using PubMed and Scopus. Studies published in English between 2015 and 2026 were screened. Eligible articles examined women in the context of any disasters and addressed fertility behavior, pregnancy outcomes, reproductive and maternity care access, menstrual health, abortion care, or related psychosocial outcomes. Editorials, commentaries, letters, conference abstracts, and studies without clear methodology were excluded. The final synthesis was organized thematically. Results: The evidence showed that disasters are associated with altered fertility behavior, increased pregnancy complications, disrupted prenatal and maternity care, reduced access to reproductive health services, and heightened psychosocial vulnerability. Fertility responses varied by disaster type and context, while obstetric risks were elevated after power outages, hurricanes, floods, and earthquakes. Access to contraception, abortion, menstrual hygiene, and post-abortion care was frequently constrained, particularly among socially disadvantaged women. Conclusion: Disasters substantially compromise women’s reproductive health across the life course. Disaster preparedness and response should integrate gender-responsive reproductive health services as core components of resilience planning.
