A Review of Challenges and Innovations in Cloud-Based Picture Archiving and Communication Systems (Cloud-PACS)

چکیده :

Abstract: Background: Growing medical imaging data volumes are driving a paradigm shift from traditional on-premise Picture Archiving and Communication Systems (PACS) to cloud infrastructures. While Cloud-PACS efficiently traverses storage limitations and enables global teleradiology, the primary bottleneck remains limited clinical adoption due to complex data migration and security concerns. This review evaluates the intersection of cloud deployment models and Artificial Intelligence integration to ensure practical clinical viability. Method: Adhering strictly to PRISMA guidelines, a systematic search was conducted across major academic databases targeting Cloud-PACS literature. Following rigorous screening, selected foundational studies focusing on implementation architectures, AI integrations (e.g., the PACS-AI platform), and real-world deployment challenges were evaluated using MINORS criteria and included for narrative synthesis. Finding: The literature reveals a critical transition from one-size-fits-all legacy systems to sophisticated Cloud-Native microservices and Hybrid architectures. For enterprise-level deployment, tailored hybrid models currently outperform purely public architectures by mitigating absolute internet dependency and cyber risks. Furthermore, resolving the gap between theoretical research and healthcare implementation necessitates a comprehensive evaluation; integrating clinical, organizational, technological, and policy dimensions (the Four-Pillar Framework) is now mandatory to bridge the gap between conceptual architectures and clinical application. Conclusion: Cloud-PACS has evolved from a novel storage alternative into an indispensable foundational infrastructure for AI-driven diagnostics and global teleradiology. By adopting tailored architectures and enforcing strict interoperability standards, these systems are establishing robust imaging pipelines poised to drastically reduce infrastructure bottlenecks and accelerate modern healthcare delivery.
