چکیده :
Background: The prevalence of chronic pain and anxiety is increasing in older adults, and pharmacological treatments have limitations. Mindfulness based interventions (MBIs), including Mindfulness Based Stress Reduction (MBSR) and Acceptance and Commitment Therapy (ACT), have emerged as non pharmacological approaches. Although previous studies have addressed the role of MBIs in chronic pain and anxiety separately, the combined role of telehealth and MBIs in treating these disorders in older adults has not been directly investigated Methods: MeSH terms (e.g., "Mindfulness", "Chronic Pain", "Telemedicine") and free keywords were used to search PubMed, Scopus, and ScienceDirect (2018-2025). Randomized controlled trials, meta analyses, and systematic reviews were included, with particular attention to studies examining both in person and telehealth modalities. Findings: Recent studies show that MBIs significantly reduce anxiety severity, and pain intensity in older adults. Telehealth MBIs (online group programs and self paced versions) are effective. Evidence from the LAMP randomized clinical trial demonstrates that telehealth delivered mindfulness interventions improve pain related functional outcomes. Additionally, studies on telehealth delivered mindfulness meditation, cognitive therapy, and behavioral activation for chronic low back pain have shown improvements in pain management. Regarding mechanisms, mindfulness reduces brain reactivity to pain and modulates stress related systems. Notably, evidence specifically examining the synergistic effect of telehealth delivery and mindfulness content in older adults with co occurring chronic pain and anxiety is scarce. Age differences in the effectiveness of telehealth MBIs for chronic pain further underscore the need for geriatric specific research. Conclusion: MBIs are safe and effective for reducing anxiety and improving chronic pain management in older adults. Telehealth methods are particularly valuable for those with mobility limitations. However, given that most existing telehealth MBI studies have focused on younger cohorts or single condition populations, larger trials of telehealth MBIs in older adults are needed.
