چکیده :
Background: Chronic wounds, including diabetic foot ulcer, continue to be a significant clinical challenge due to the inability of tissue repair. Evidence suggests that wound healing is regulated by the coordinated interaction of neural, immune, mechanical, and metabolic signaling networks rather than isolated biological pathways (1, 2). This review explored the emerging concept of neuro-immune and mechano-metabolic crosstalk as an integrated framework for understanding chronic wound pathophysiology and advancing regenerative medicine. Methods: The present narrative review study was conducted by synthesizing recent experimental, translational, and clinical evidence on neuro-immune regulation, mechano-transduction, immuno-metabolism, and regenerative therapies relevant to chronic wound healing. Particular emphasis was placed on molecular mechanisms linking neuronal signaling, immune cell function, extracellular matrix mechanics, and cellular metabolism (2-4). Findings: Neuro-immune mediators regulate inflammation, angiogenesis, fibroblast activation, and re-epithelialization, while mechano-metabolic pathways coordinate cellular adaptation to biomechanical and metabolic stress (2, 3, 5). Dysregulation of these pathways drives persistent inflammation, cellular senescence, impaired extracellular matrix remodeling, and defective tissue regeneration. Multi-omics approaches are enabling precision therapies, including immunomodulatory biomaterials, extracellular vesicles, stem cell therapies, neuromodulation, and mechanoresponsive scaffolds (3, 5). Conclusion: Integrating neuro-immune and mechano-metabolic biology offers a systems-level view of chronic wound healing and a foundation for regenerative medicine. Targeting these interconnected pathways can accelerate the tissue repair process and reduce the likelihood of wound recurrence.
