چکیده :
Background: The prevailing “glucose-centric” paradigm in the management of Type 2 Diabetes Mellitus (T2DM), largely centered on HbA1c, does not fully account for the disease’s multisystemic complexity. The concept of allostatic load defined as the cumulative physiological burden imposed by chronic stress offers an integrative framework that connects neuroendocrine, inflammatory, and autonomic dysregulation with the underlying pathophysiology of diabetes. Objective: This review aims to synthesize current evidence regarding the role of allostatic load in T2DM, describe practical approaches for its clinical assessment, and propose an integrated healthcare model for its systematic management. Methods: A structured literature search was conducted in PubMed/MEDLINE, Scopus, and Web of Science covering the period from 2008 to 2026. Findings: Elevated allostatic load has been consistently associated with an increased risk of T2DM, greater prevalence of microvascular and macrovascular complications and diminished quality of life. Clinically, assessment may be operationalized through a two-stage strategy combining validated psychometric instruments with objective biomarkers. We propose a four-pillar integrated care model comprising: (1) digital profiling and incorporation of a composite allostatic load score into electronic health records; (2) coordinated intervention by a multidisciplinary team; (3) delivery of targeted, mechanism-based therapies and (4) continuous monitoring with adaptive personalization through digital health technologies. Conclusion: Incorporating allostatic load into routine diabetes care represents a conceptual shift from reactive glycemic management toward proactive enhancement of physiological resilience. By addressing systemic dysregulation at its roots, this framework provides a promising pathway toward improved clinical outcomes and a more preventive, sustainable model of healthcare delivery.
