چکیده :
Introduction: Cancer is a leading cause of morbidity and many of the symptoms that patients have after cancer can be physical or psychosocial and long-term, limiting their health-related quality of life.(1,2) Supportive/palliative care services are increasingly recommended to be integrated with cancer care, but the extent of this integration is inconsistent.(4,5) The purpose of this review was to provide a summary of evidence of the effectiveness of these services in reducing suffering and enhancing the quality of life for adults with cancer. Methods: Narrative literature review was conducted by searching the PubMed/MEDLINE, Embase, Scopus and Cochrane Library databases for literature published since 2014 through 2026, along with seminal randomized trials and guidance documents. Randomized controlled trials, systematic reviews, meta-analyses, cohort studies and guideline/consensus statements in adults with cancer were eligible sources. Narrative synthesis of findings was the most appropriate method for this because interventions, populations and outcomes were very different. Results: The findings suggest that interdisciplinary supportive and palliative care can help to manage symptom burden, emotional distress, social needs, communication and spiritual concerns.(2,4,5) Meta-analyses and systematic reviews have indicated that palliative care may enhance QOL and symptom management outcomes reported by patients and some evidence of benefits for caregivers.(4,5) Early palliative care was associated with better QOL, less aggressive end-of-life care, and higher mood scores in metastatic non-small-cell lung cancer; and early palliative care delivered in the outpatient setting resulted in better QOL in a cluster randomized trial for advanced cancer.(5) The Cochrane review indicated potential for improvements in quality of life and symptom severity, but the results were somewhat uncertain because of the heterogeneity. Supportive care was also found to be associated with fewer emergency visits, less intensive end-of-life care, and fewer in-hospital deaths when delivered in line with patient preferences, and to be associated with less toxicity to treatment and increased tolerance.(4,5) Discussion: The evidence presented collectively demonstrates that early integrated supportive and palliative oncology services have a positive effect on patient outcomes and may decrease the burden of the caregiver and health care utilization.(4,5) Barriers that remain are access, delayed referral, lack of staff, and incorrect perceptions about palliative care.(4,5) The challenge isn't whether these services are effective, but how they can be effective and provide equitable care at every point in the cancer experience.
