چکیده :
Benign paroxysmal positional vertigo (BPPV) is the most common peripheral vestibular disorder. Although its diagnosis and treatment are well established, the role of cardiometabolic factors in BPPV remains unclear. Evidence on these associations in the Iranian population is limited. This study investigated selected cardiometabolic and related clinical factors associated with BPPV. Method: This case-control study included 100 participants, comprising 50 patients with BPPV and 50 age- and sex-matched controls, recruited from the Neurology Clinic of Shariati Hospital, Isfahan, Iran, during 2025-2026. BPPV was diagnosed based on clinical assessment by a specialist and/or a positive Dix-Hallpike test. Data on cardiometabolic factors, including weight, fasting blood glucose, antihypertensive medication use, and lipid-lowering medication use, as well as selected clinical and lifestyle factors, were collected for both groups. The groups were compared using appropriate statistical tests for continuous and categorical variables. A P value <0.05 was considered statistically significant. Finding: The BPPV group had a higher mean weight compared with the control group (81.28 ± 6.68 vs. 74.08 ± 5.49 kg, P<0.001). Mean fasting blood glucose was also higher in patients with BPPV (122.84 ± 18.72 vs. 95.28 ± 9.07 mg/dL, P<0.001). Antihypertensive medication use was reported in 68.0% of patients with BPPV compared with 30.0% of controls (P<0.001), while lipid-lowering medication use was 52.0% and 18.0%, respectively (P<0.001). Migraine history (42.0% vs. 20.0%, P=0.030) and smoking (38.0% vs. 18.0%, P=0.045) were also more common in the BPPV group. No significant difference was found for family history of BPPV or alcohol consumption. Conclusion: In this study, BPPV was associated with higher weight and fasting blood glucose levels, as well as greater use of antihypertensive and lipid-lowering medications. These findings suggest that cardiometabolic factors may be associated with BPPV. Further studies with larger sample sizes and prospective designs are needed to better understand these associations and their clinical relevance.
